Well, from what I have seen, opening in google docs messes with the format, so my recommendation is to download and open in Word. As for filling it out, you are just meant to click in each of the boxes you want to fill out, and it should let you type freely. Same with the description section below, should be able to paste images or type there by clicking. If you still have trouble, feel free to just send me your entry in your own document, and label that it is a custom version in the tag of the email.
